For EtG, additional tests, and/or those exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale for weighing. If scalp hair cannot be collected, an equivalent amount of body hair can be used. Head hair pertains exclusively to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary phases in processing a drug test in the laboratory include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary handling of a sample within a laboratory’s framework. This entails ensuring the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data input absent from an electronic chain of custody record.

Screening constitutes a preliminary rapid assessment for drugs. Despite its cost-effectiveness in excluding drug use for most samples, a positive screening result must be verified for court admissibility. Every sample that is assumed positive in Screening requires an additional confirmation step.

Should a sample be presumed positive in Screening, additional hair is extracted from the initial specimen for Extraction. In this phase, drugs are removed from hair at significantly lower levels in comparison to other approaches (e.g., urine or oral fluid), which makes hair drug analysis the most complex technique to execute.

Verification of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples undergo a cleansing cycle before confirmation as necessary. The end-to-end laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair classification and adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
