What it is, How it works

When collected near the scalp, hair can offer a detection window of up to about three months for both alcohol and other substances. Hair collection is straightforward, relatively hard to tamper with, and easy to transport.

A 1.5-inch specimen of roughly 200 strands of hair (similar in size to a #2 pencil) closest to the scalp produces 100mg of hair, which is an ideal amount for screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg of the specimen is suggested. We advise using a jeweler’s scale to weigh the sample. If scalp hair is not an option, a comparable amount of body hair can be obtained. When referring to head hair, we mean only scalp hair. Body hair includes all other hair varieties (such as facial and axillary).

Process Summary

The key steps in the laboratory testing for a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial sample processing into the lab system. This step checks that the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and finishes any extra data entry not given by an electronic chain of custody system.

Screening involves an initial evaluation for substances of abuse. While Screening is a cost-effective strategy to exclude drug consumption in most samples, a positive screen necessitates further confirmation for legal admissibility. Any samples that test presumptively positive during Screening require a secondary confirmation.

If a sample tests presumptively positive during the Screening phase, more hair will be extracted from the initial sample and undergo preparation for Extraction. Here, drugs are extracted from the hair at a much lower concentration relative to other methods (e.g., urine or oral fluid), making hair drug tests the most challenging methodology to conduct.

Confirmation of any positive result from Screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as needed. The entire lab process from Accessioning to Confirmation is supervised under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
