In the absence of scalp hair, an equal quantity of body hair suffices. "Head hair" pertains exclusively to scalp hair, while "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The main stages within laboratory processing of drug test outcomes are Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the initial integration of a sample into the laboratory’s system. This process ensures the sample is sealed and delivered correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data not covered by an electronic chain of custody system.

Screening constitutes an initial rapid assessment for substance abuse. While it's an economical method to eliminate drug usage in most samples, any positively screened samples must be validated for court acceptance. All presumptively positive samples via Screening necessitate a follow-up confirmation.

In cases where Screening indicates a potential positive, extra hair is extracted from the specimen for Extraction. Here, substances are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), highlighting the challenges of hair drug testing.

Confirmation for any positive screened sample employs GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positives undergo a wash if necessary. The entire process from Accessioning through Confirmation is reviewed under both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
