What it is, How it works

A 1.5-inch sample consisting of about 200 strands of hair (roughly the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is suggested. We recommend using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, a similar quantity of body hair may be collected. Reference to head hair specifically means scalp hair. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The four fundamental stages in the laboratory analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a lab’s system. This entails ensuring the sample was properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening is an initial rapid examination for substances of abuse. While Screening is a cost-effective means to rule out drug consumption in most samples, a positive screen necessitates confirmation to be legally admissible. Any samples presumptively positive in Screening require a secondary confirmation.

If a sample shows presumptive positivity during Screening, additional hair is extracted from the initial specimen and prepped for Extraction. During this phase, drugs are extracted from hair at a considerably lower concentration compared to other methodologies (e.g., urine or oral fluid), contributing to why hair drug testing is the most challenging methodology to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive specimens are washed as necessary. The complete laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
