A hair sample of 1.5 inches, comprising approximately 200 strands (about the size of a #2 pencil), collected nearest to the scalp provides 100mg, the optimal quantity for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg specimen is advisable. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is not available, an equal amount of body hair may suffice. References to head hair pertain exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The core stages in the laboratory analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of integrating a sample into a lab's system. This procedure entails verifying the proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any outstanding data entry not covered by an electronic chain of custody.

Screening is a preliminary check for illegal drugs. While Screening is cost-efficient for eliminating drug intake in most samples, a positive screening needs corroboration for court admittance. Any samples flagged as presumptive positives in Screening require subsequent confirmation.

If Screening indicates a presumptive positive, additional hair is extracted from the initial sample for the Extraction phase. Here, drugs are isolated from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Confirmation of any positive screening result employs GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive are thoroughly cleaned prior to confirmation if necessary. The entire lab procedure from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
