What it is, How it works

When extracted near the scalp, hair can provide up to about a 3-month detection period for alcohol and other drugs. Hair is straightforward to collect, relatively hard to adulterate, and convenient to ship.

A 1.5-inch segment of roughly 200 strands of hair (around the diameter of a #2 pencil) closest to the scalp will yield 100mg of hair, the perfect sample for screening and confirmation. For EtG, add-ons, and/or assessments above 10 panels, 150mg of the specimen is suggested. We advise weighing the specimen on a jeweler’s scale. If scalp hair is unavailable, the same quantity of body hair may be collected. When mentioning head hair, we mean scalp hair exclusively. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The four primary steps involved in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into a lab’s system. This involves verifying that the sample was sealed and shipped properly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening consists of an initial rapid check for drugs of abuse. While Screening is an economical approach to rule out drug usage on most samples, a positive screen needs to be confirmed to be admissible in court. Samples that are presumptively positive in Screening necessitate secondary confirmation.

If a sample is presumptively positive in the Screening stage, more hair is obtained from the initial specimen and prepared for Extraction. At this stage, drugs are extracted from hair at a substantially lower concentration than in other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most intricate methodology to conduct.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
