What it is, How it works

A sample measuring 1.5 inches and consisting of roughly 200 hair strands (around the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal amount for screening and verification. For EtG, add-ons, and/or tests exceeding 10 panels, it is advised to have 150mg of the specimen. It is suggested to weigh the specimen using a jeweler’s scale. In the absence of scalp hair, a similar amount of body hair may be obtained. By head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The principal steps in the laboratory analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample into a laboratory's framework. This step includes confirming the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening refers to an initial swift evaluation for drugs of abuse. While Screening is an economical approach to eliminate drug use suspicion in the majority of samples, a positive screen needs verification to be valid in court. Any samples that are deemed presumptively positive in Screening necessitate a follow-up confirmation.

Should a sample test presumptively positive during the Screening phase, additional hair is removed from the original specimen to prepare for Extraction. At this stage, drugs are extracted from hair at significantly lesser concentrations than in other methods (ex. urine or oral fluid), which is why hair drug screening is considered the most complex methodology to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed prior to confirmation as required. The entire lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
