A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil's thickness) nearest to the scalp yields 100mg, the perfect amount for both screening and confirmation tests. For EtG, additional tests, and/or tests exceeding 10 panels, a sample of 150mg is suggested. It's advisable to use a jeweler's scale for weighing the sample. In cases where scalp hair cannot be collected, an equivalent quantity of body hair may be obtained. When we reference head hair, it specifically means scalp hair. Body hair pertains to all other hair variations (such as facial and axillary hair).

Process Overview

The process for handling a drug testing result in a lab involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails initiating a sample's integration into a laboratory's system, including ensuring the sample was correctly sealed and transported, allocating a unique LAN (Laboratory Accessioning Number), and completing any additional required data input not available through an electronic chain of custody process.

Screening comprises an initial rapid assessment of drug use. While cost-efficient for excluding drug use in most cases, a positive result here necessitates confirmation to be valid in legal settings. Samples with presumptive positives during Screening require a confirmatory test.

If there's a presumptive positive in the Screening phase, extra hair is drawn from the original sample for the Extraction process. Here, drugs are separated from the hair at lower concentrations compared to other techniques (such as in urine or oral fluid), rendering hair drug screening the most intricate methodology.

GC/MS, GC/MS/MS, or LC/MS/MS methods confirm any positive screening outcomes. Presumptive positives are cleaned before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
