For EtG, add-ons, and tests with more than 10 panels, 150mg of hair is suggested. We advise measuring the sample using a jeweler's scale. If scalp hair is not present, an equivalent amount of body hair may be used. "Head hair" specifically means "scalp hair," while "body hair" includes all other hair types (such as facial, axillary, etc.).

Process Overview

There are four primary steps in laboratory drug test processing: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of integrating a sample into a laboratory's workflow. This process includes checking the seal and shipment of the sample, allocating a random LAN (Laboratory Accessioning Number), and completing missing data entry outside of an electronic chain of custody system.

Screening performs a preliminary swift evaluation for substance abuse. Although Screening is a cost-effective means to exclude drug presence in most cases, any positive findings during this phase require confirmation for legal proceedings. Samples that show presumptive positives during Screening need subsequent verification.

When Screening suggests a positive result, additional hair is extracted from the original sample for the Extraction phase. During Extraction, drugs are removed from hair at a much smaller scale than in other methods (such as urine or oral fluid), which makes hair drug screening a challenging technique.

Verification of any positive Screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo cleansing before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation undergoes scrutiny under both CAP (College of American Pathologists) Hair criteria and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
