What it is, How it works

When taken near the scalp, hair can indicate drug and alcohol use for up to a 3-month period. Collection is straightforward, tampering is challenging, and transportation is uncomplicated.

A sample of 1.5 inches comprising approximately 200 hairs (roughly the thickness of a #2 pencil) near the scalp produces 100mg of hair, which is optimal for analysis. For EtG, additional tests, and/or analyses exceeding 10 panels, a 150mg specimen is advised. We suggest weighing the sample on a jeweler’s scale. If scalp hair cannot be obtained, a corresponding quantity of body hair can be gathered. References to head hair specifically pertain to scalp hair. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

There are four key stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially integrated into a lab’s system. This phase includes ensuring the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any further data input not covered by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening is an economical approach to eliminate drug use in most cases, a positive result must be confirmed for legal acceptance. Samples that are preliminarily positive in Screening necessitate an additional confirmation.

If Screening suggests a sample is positive, more hairs are extracted from the original specimen for the Extraction phase. Here, drugs are isolated from hair at a significantly reduced concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging technique to execute.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Samples with presumptive positives undergo washing before Confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
