It is straightforward to gather, relatively hard to tamper with, and convenient for shipping.

A sample measuring 1.5 inches comprising approximately 200 hair strands (about the size of a #2 pencil) near the scalp will yield 100mg of hair, which is optimal for screening and validation. For EtG, supplementary evaluations, and/or tests exceeding 10 panels, 150mg of the sample is suggested. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not attainable, a corresponding quantity of body hair can be obtained. When we mention head hair, we specifically mean scalp hair only. Body hair includes other types of hair (facial, axillary, etc.).

Procedure Summary

The laboratory process for a drug test outcome involves four crucial stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample into a lab's system. This step checks if the sample was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and finalizes any outstanding data entry not covered by an electronic chain of custody system.

Screening constitutes a preliminary rapid test for drugs of abuse. Though Screening offers a budget-friendly method to exclude drug use for most samples, a screen indicating drug presence needs verification to be court-admissible. Any samples that test positive during Screening require an additional confirmation.

If a sample shows presumptive positivity during Screening, additional hair is extracted from the original sample and readied for Extraction. During this phase, drugs are obtained from hair at significantly lower concentrations than seen in other methods (e.g., urine or oral fluid), making hair drug analysis the most challenging to perform.

Confirmation of any positive result from screening occurs through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples undergo necessary washing. The full laboratory sequence from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
