A 1.5-inch sample, around the width of a #2 pencil, of about 200 hair strands closest to the scalp, equates to 100mg of hair, which is the optimal sample size for screening and confirmation. If testing for EtG, add-ons, and/or more than 10 panels, a 150mg sample is advised. We suggest using a jeweler's scale to weigh the specimen. If scalp hair cannot be obtained, an equivalent quantity of body hair is also acceptable. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

Four primary stages are engaged in the laboratory analysis of a drug test result, which are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves entering a sample into a laboratory system initially. This ensures the sample was correctly sealed and dispatched. It includes the assignment of a random LAN (Laboratory Accessioning Number), along with finalizing any supplementary data entry absent in an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Although Screening offers a cost-efficient method to eliminate drug usage in most samples, a positive result must be verified to be legally valid. Any samples that test presumptively positive during Screening necessitate secondary confirmation.

If the Screening stage indicates a presumptive positive result, additional hair is extracted from the original sample for Extraction. Drugs are isolated from hair at considerably lower concentrations than with other techniques (e.g., urine or oral fluid), hence making hair drug screening one of the most challenging methodologies.

Positive screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation when necessary. The holistic laboratory process from Accessioning to Confirmation is regulated under both the CAP (College of American Pathologists) Hair designation and accreditation standards of ISO / IEC 17025.
