A sample measuring 1.5 inches and consisting of roughly 200 hair strands (approximating the diameter of a #2 pencil) near the scalp can provide 100mg of hair, which is optimal for screening and verification. For EtG, additional tests, and/or analyses exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, an equivalent volume of body hair may be used. References to head hair refer exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the introductory integration of a sample into the laboratory system. This includes affirming the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent from an electronic chain of custody system.

Screening is a preliminary examination for commonly abused drugs. While Screening is a cost-efficient method for excluding drug use in the majority of cases, a positive screen must be verified for admissibility in legal proceedings. Any samples that are tentatively positive in Screening require further confirmation.

Should a sample be tentatively positive during Screening, additional hair is collected from the original sample and prepared for Extraction. In this phase, drugs are extracted from hair at a much lower concentration than in other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation if necessary. The entire lab process from Accessioning to Confirmation is subjected to review under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
