What it is, How it works

Hair analysis has gained recognition as an effective method for spotting substance and alcohol consumption. Hair archives a long-term record of alcohol and other substances by enclosing biomarkers within the hair fiber. Proximate to the scalp, hair can present a detection window of roughly 3 months for alcohol and other substances. The simplicity of collecting hair, its resistance to tampering, and its ease of shipping make it advantageous.

A sample size of 1.5 inches, consisting of approximately 200 hair strands (about the diameter of a #2 pencil) nearest to the scalp, provides 100mg of hair, the optimal amount for both screening and validation. For EtG, additional markers, and/or testing exceeding 10 panels, it is suggested to gather 150mg of the sample. Using a jeweler’s scale for specimen measurement is advised. If scalp hair is unobtainable, an equivalent amount of body hair may be collected. When specifying head hair, it refers strictly to scalp hair. Body hair signifies all other hair types (facial, axillary, etc.).

Process Overview

There are four fundamental stages in processing a test result in a laboratory: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the preliminary handling of a sample into the laboratory system. This phase includes confirming the sample was adequately sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry not covered by an electronic custody system.

Screening consists of an initial rapid assessment for abuse substances. Although Screening offers a cost-effective means to exclude substance usage for many samples, a positive screen must be confirmed for court admissibility. All samples provisionally positive in Screening are subjected to further confirmation.

If a Screening stage yields a presumptively positive result, additional hair is extracted from the primary specimen for Extraction. During Extraction, drug concentrations are isolated from hair at levels lower than those of other techniques (e.g., urine or saliva), making hair drug analysis a more intricate procedure.

Any positive initial screen is affirmed through GC/MS, GC/MS/MS, or LC/MS/MS procedures. Prior to confirmation, all presumptive positive samples undergo washing as necessary. Every step of the lab process from Accessioning to Confirmation is reviewed, complying with the CAP (College of American Pathologists) Hair standards and accredited to ISO / IEC 17025 criteria.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
