What it is, How it works

When cut close to the scalp, hair offers an approximate 3-month detection period for alcohol and drugs. Hair is straightforward to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch section of nearly 200 hair strands (similar to the size of a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal sample for both screening and confirmation. For EtG, add-ons, and tests exceeding 10 panels, 150mg of hair is advised. We suggest weighing the hair on a jewelry scale. If scalp hair isn't available, a similar quantity of body hair can be collected. When mentioning head hair, we are exclusively discussing scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four essential steps in the laboratory evaluation of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample within a lab’s system. This involves confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry that an electronic chain of custody system does not cover.

Screening consists of an initial quick examination for drugs of abuse. Although Screening is a budget-friendly method to exclude drug use for most samples, a positive screen must be confirmed to be legally valid. All samples that show a presumptive positive in Screening require additional confirmation.

If a sample is deemed presumptively positive during the Screening stage, additional hair is extracted from the primary specimen and prepared for Extraction. In this phase, drugs are isolated from hair at a notably lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a presumptive positive are washed before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
