A sample measuring 1.5 inches, containing roughly 200 hair strands (similar to the width of a #2 pencil) near the scalp will provide 100mg of hair, which is the optimal amount for analysis. For EtG, additional tests, and/or panels exceeding 10, collecting 150mg of the sample is advisable. It is best to use a jeweler’s scale for measuring the specimen. Should scalp hair not be available, a similar quantity of body hair may be collected. In discussing head hair, we solely refer to scalp hair. Body hair encompasses all other types (facial, arm, etc.).

Process Overview

Four key phases are involved in processing drug test results in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary handling of a sample within a laboratory’s system. This step involves ensuring the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and filling in any further data entry not covered by an electronic chain of custody system.

Screening consists of a preliminary, rapid assessment for drug misuse. Although Screening is an economical approach for dismissing drug usage for most samples, a positive screen must be verified for legal acceptability in court. Samples that test tentatively positive in Screening necessitate additional confirmation.

If Screening indicates a sample is tentatively positive, extra hair is pulled from the primary sample for Extraction preparation. This phase involves extracting drugs from hair at significantly lower concentrations than other methods (such as urine or oral fluid), which is why hair drug testing is the most intricate method to execute.

Verification of any positive screening outcome is executed via GC/MS, GC/MS/MS or LC/MS/MS. All samples testing presumptively positive are cleansed before confirmation as required. The complete lab process from Accessioning to Confirmation is evaluated following both the CAP (College of American Pathologists) Hair framework and the accreditation to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
