If scalp hair is not available, a similar quantity of body hair can be obtained. When mentioning head hair, we are specifically addressing scalp hair. Body hair encompasses all other forms of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug testing result is broken down into four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the foundational processing of a sample into the laboratory's framework. This step ensures that the sample was correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and fulfills any extra data entry requirements not catered by an electronic chain of custody protocol.

Screening is an initial rapid assessment for drug substances. Although it serves as a cost-efficient means to exclude drug usage in most samples, a positive screening result necessitates confirmation for legal admissibility. Any sample deemed presumptively positive in Screening mandates a follow-up verification.

In cases where a sample shows presumptive positivity during the Screening, additional hair is extracted from the sample for the Extraction phase. At this stage, drugs are isolated from hair at significantly lower concentrations than other techniques (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples marked as presumptive positive are cleansed before confirmation as required. The entire laboratory sequence from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
