A sample that is 1.5 inches in length and contains approximately 200 hair strands (about the diameter of a #2 pencil) near the scalp will yield 100mg of hair, which is the optimal amount for both initial screening and confirmation. For EtG and additional analyses, or for tests exceeding ten panels, a 150mg sample is advisable. It is suggested to weigh the sample with a jeweler’s scale. If scalp hair cannot be obtained, the equivalent amount of body hair can be used. When referring to head hair, we mean specifically scalp hair. Body hair encompasses all other hair forms (such as facial or underarm hair).

Procedure Synopsis

The laboratory steps for processing a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of inputting a sample into a laboratory system for the first time. This phase includes checking that the sample was appropriately sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and entering any additional data not available through an electronic custody system.

Screening is an initial assessment for the presence of drugs. While Screening is an economical way to dismiss the presence of drugs in most samples, a positive result must be validated for legal proceedings. Samples that return a presumptive positive in Screening require subsequent confirmation.

If a sample indicates a presumptive positive during Screening, additional hair is taken from the primary sample and prepared for Extraction. During Extraction, drugs are isolated from hair at significantly lower concentrations compared to alternatives like urine or oral samples, underscoring why hair drug testing is among the most challenging methodologies.

Confirmation of any positive Screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples commonly undergo washing before confirmation when needed. The entire testing sequence from Accessioning to Confirmation is overseen under both CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation.
