What it is, How it works

When cut near the scalp, hair can offer a detection period of up to roughly 3 months for alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and easy to transport.

A 1.5-inch segment comprising around 200 hairs (about the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal sample size for both initial testing and confirmation. For EtG, additional tests, and/or panels beyond 10, a quantity of 150mg of the specimen is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair is inaccesible, an equivalent volume of body hair may be utilized. In terms of head hair, we specifically mean scalp hair. Body hair pertains to all other kinds of hair (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result encompasses the following four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning focuses on integrating a sample into a lab’s system. This procedure entails confirming that the sample was correctly sealed and conveyed, allocating a unique LAN (Laboratory Accessioning Number), and finalizing any supplementary data not included via an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drug abuse. Although Screening is a cost-efficient approach for excluding drug use in the majority of samples, a confirmed positive screen is necessary for legal proceedings. Samples that show a presumptive positive in Screening mandate further validation.

Should a sample indicate a presumptive positive during Screening, additional hair is taken from the initial sample and prepared for Extraction. This step involves extracting drugs from hair at a considerably reduced concentration compared to other methods (i.e., urine or oral fluid), thus making hair drug testing the most challenging methodology to execute.

Any positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all samples with presumptive positives are washed as required. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
