The main advantage of a hair follicle test lies in its extended detection window for up to 90 days, monitored collection process, and resistance to tampering. However, for detecting recent drug use within the past 5 days, urine tests remain the most precise.

What it is, How it works

Hair testing has gained recognition as a robust method for identifying drug and alcohol consumption. By capturing biomarkers within the fibers of hair as it grows, hair provides a long-term record of drug and alcohol intake. When collected from near the scalp, hair offers a detection window of up to around three months for alcohol and drug presence. Hair is straightforward to collect, relatively hard to tamper with, and easy to send.

A 1.5-inch sample consisting of approximately 200 hair strands (equivalent to the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal amount for testing and verification. For EtG testing, additional tests, or panels beyond 10, 150mg of hair is recommended. We advise using a jeweler’s scale to weigh the hair. If scalp hair is not available, the same quantity of body hair can be used. Here, head hair refers specifically to scalp hair. Body hair encompasses all other hair types (like facial, axillary hair).

Process Overview

The primary phases in processing drug test results in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage where a sample is integrated into a lab's system. This step includes confirming that the sample was adequately sealed and transported, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing further data entry not covered by an electronic chain of custody system.

Screening entails a swift preliminary scan for drugs of abuse. Although Screening is a cost-effective means of eliminating drug use in most samples, a positive result requires confirmation for legal acceptability. Any samples testing positive in Screening must undergo secondary confirmation.

When a sample tests presumptively positive during Screening, additional hair is extracted from the initial sample for the Extraction phase. Here, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening a challenging methodology.

Confirmation of any positive result from Screening is done through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed if necessary. The full lab process from Accessioning to Confirmation is inspected both under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
