What it is, How it works

When cut near the scalp, hair can reflect up to an estimated 3-month detection span for both alcohol and drugs. Hair is straightforward to collect, relatively tamper-resistant, and simple to ship.

A 1.5-inch section of about 200 strands of hair (approximately the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample amount for screening and verification. For EtG, add-ons, and/or tests including more than 10 panels, it is advised to use 150mg of the sample. We suggest weighing the sample on a jeweler's scale. Should scalp hair be unavailable, an equivalent quantity of body hair can be gathered. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial entry of a specimen into the lab's system. This includes checking that the specimen was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not managed by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs. While Screening is cost-efficient for excluding drug use in the majority of samples, a positive screen must be verified to be acceptable in a legal setting. Samples that are presumptively positive in Screening need supplementary confirmation.

Should a sample be presumptively positive during Screening, additional hair is taken from the original sample and prepared for Extraction. At this stage, drugs are extracted from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The full lab procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
