In cases where scalp hair is not accessible, an equivalent amount of body hair may be gathered. When mentioning head hair, it specifically refers to scalp hair. Body hair pertains to other types of hair (such as facial or axillary hair).

Process Overview

The laboratory evaluation of drug test outcomes encompasses four pivotal phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial sample handling within a laboratory framework. Tasks include confirming proper sealing and shipment of the sample, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry omitted by an electronic chain of custody system.

Screening serves as an initial rapid assessment for substance abuse. Although Screening is an economical strategy for excluding drug consumption in most samples, a positive result necessitates confirmation for legal acceptability. Specimens deemed presumptively positive in Screening require additional validation.

If Screening indicates a presumptive positive, further hair amounts are extracted from the original sample for Extraction. Here, drugs are isolated from hair at markedly lower concentrations than other procedures (such as urine or oral fluid), making hair drug analysis one of the most challenging methodologies to execute.

Verification of any positive screen result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo washing before confirmation when necessary. The complete laboratory procedure, from Accessioning through Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
