A 1.5-inch section containing about 200 strands (akin to a #2 pencil thickness) closest to the scalp supplies 100mg, which is the ideal amount for screening and verification. For EtG, additional panels, or over 10 panels, 150mg is advised. It's suggested to weigh the hair using a jeweler’s scale. If scalp hair is lacking, an equivalent amount of body hair may be used. Mention of head hair pertains only to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary stages included in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial integration into the lab system. This covers confirming the sample’s proper sealing and shipping, assigning it a unique LAN (Laboratory Accessioning Number), and finalizing any extra data needed, not available through the electronic chain of custody.

Screening conducts a preliminary rapid assessment for drugs of abuse. Though Screening is economical for ruling out drug use in most samples, a positive result must be confirmed for court admissibility. Samples testing presumptive positive in Screening need further confirmation.

For samples flagged as presumptive positive in Screening, extra hair is extracted for the Extraction process. Here, drugs are extracted from hair at much lower densities than other methods (such as urine or oral fluids), making hair testing more challenging.

Confirmation for positive screenings is done with GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive readings are cleansed prior to confirmation as needed. The entire process from Accessioning to Confirmation undergoes review according to CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
