For EtG, additional panels, or more than 10-panel tests, 150mg of the sample is advised. It is suggested to weigh the sample using a jeweler's scale. If scalp hair isn’t obtainable, a comparable quantity of body hair might be used. The term "head hair" specifically refers to scalp hair, while "body hair" covers all other hair types (such as facial, axillary, etc.).

Process Overview

The four primary phases in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial incorporation of a sample into a lab's system. This step verifies the sample was properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not supplied by an electronic chain of custody system.

Screening includes an initial rapid assessment for substances of abuse. Though Screening is a cost-effective technique for eliminating drug use in most samples, a positive screen must be confirmed for it to be court-admissible. Samples with presumptive positive results during Screening require additional confirmation.

For presumptive positive results in the Screening phase, more hair is extracted from the original sample and readied for Extraction. During this phase, drugs are removed from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), which makes hair drug screening the most complex method to conduct.

All positive Screening results proceed to Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed prior to confirmation when necessary. The laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standard accreditations.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
