A 1.5-inch portion of around 200 hair strands (comparable to a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for screening and verification. For EtG, additional tests, and/or tests beyond 10 panels, a specimen size of 150mg is advised. We suggest weighing the specimen with a jeweler’s scale. If scalp hair is not present, an equivalent amount of body hair can be collected. When mentioning head hair, we specifically refer to scalp hair only. Body hair pertains to all other types (facial, axillary, etc.).

Procedure Summary

The four primary steps in laboratory drug testing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into a lab's system. This process includes ensuring the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry not covered by an electronic chain of custody system.

Screening is an initial rapid assessment for substance abuse. While Screening is an economical method to eliminate drug usage in the majority of cases, a positive screen must be validated to be legally acceptable. Samples that show presumptive positives in Screening do need further confirmation.

If a specimen shows presumptive positivity in the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. In this step, drugs are isolated from hair at much lower levels than in other methodologies (for instance, urine or oral fluid), which is why hair drug testing is the most complex method to conduct.

Verification of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation if necessary. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair categorization and the accreditation to ISO / IEC 17025 benchmarks.
