What it is, How it works

The strands of hair capture long-term evidence of drug and alcohol usage by embedding biomarkers within the fiber. When hair samples are collected from near the scalp, they can offer a detection range of up to about 90 days for alcohol and drug use. The process of collecting hair samples is straightforward, challenging to tamper with, and easy to transport.

Taking a 1.5-inch section from the scalp with roughly 200 hair strands (similar in size to a #2 pencil) provides 100mg of hair, which is the perfect amount for both testing and verification. For EtG addons or screenings with more than 10 panels, it’s advisable to gather 150mg of hair. It's best to measure the sample using a jeweler’s scale. If scalp hair cannot be obtained, a comparable amount of body hair can be used. In this context, "head hair" specifically means "scalp hair", whereas "body hair" includes all other types like facial and axillary hair.

Process Overview

The cornerstone procedures for laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is first processed into the lab’s system by verifying proper sealing and shipping, assigning it a Laboratory Accessioning Number (LAN), and entering additional information beyond what the electronic chain of custody indicates.

In the Screening phase, an initial simple assay checks for abusive substances. While Screening is a cost-effective method to eliminate drug presence in most samples, a confirmed positive result is necessary for legal reliability. Any sample that yields a presumptive positive during Screening must undergo secondary confirmation.

Upon a presumptive positive in Screening, additional hair is extracted from the original sample for the Extraction process. Here, drugs are isolated from hair at significantly lower concentrations than with other testing methods like urine or oral fluid, underlining why hair drug testing is the most challenging form to conduct.

Positive results in Screening are further validated using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives go through washing before confirmation as needed. The complete lab process from Accessioning to Confirmation is evaluated under the CAP Hair guidelines and ISO / IEC 17025 standards.

The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

Biomarkers and Pharmacokinetics: The identification and validation of reliable biomarkers and pharmacokinetic studies are fundamental aspects of drug testing and analysis. The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
