For EtG enhancements or tests exceeding 10 panels, a sample of 150mg is advised. Utilizing a jeweler's scale is recommended for measurement accuracy. In cases where scalp hair is unavailable, a corresponding amount of body hair is suitable. Head hair pertains exclusively to scalp hair, whereas body hair includes all other types such as facial and axillary hair.

Process Overview

The drug test processing in the lab consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning handles the initial integration of a sample into the lab's system. It involves confirming proper sealing and shipping, assigning a random LAN (Lab Accessioning Number), and completing any supplementary data input the electronic chain of custody system might not cover.

Screening conducts a preliminary assessment for substance abuse. Although Screening is an economical way to eliminate drug use on most samples, confirmation is essential for legal acceptability in court. Presumptively positive samples in Screening necessitate additional confirmation.

For presumptively positive samples in the Screening phase, additional hair from the original sample is taken for Extraction. Drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which makes hair drug screening one of the most complex methodologies.

Confirmation of any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The complete lab procedure from Accessioning to Confirmation is assessed following the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
