By obtaining hair samples near the scalp, it is possible to assess alcohol and drug presence over a period of up to three months. Hair collection is straightforward, relatively resistant to tampering, and simple to transport.

A 1.5-inch section of approximately 200 hair strands (similar in thickness to a #2 pencil) taken nearest the scalp yields 100mg, the preferred quantity for both screening and validation. For comprehensive assessments including EtG and more than 10-panel tests, a 150mg sample is advisable. It is suggested to use a jeweler’s scale for specimen measurement. In the absence of scalp hair, an equivalent amount of body hair may be taken. "Head hair" references only scalp hair, while "body hair" encompasses all other varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test outcomes consists of four principal stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing within the lab’s system. This phase ensures the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any manual data entries absent in an electronic chain of custody system.

Screening involves an expedited evaluation for substances of abuse. Although Screening is a cost-efficient means to exclude drug usage for most samples, a positive indication requires confirmation before being acknowledged in legal contexts. All samples that test positive during Screening necessitate further confirmation.

Should a sample test positive in the Screening phase, additional hair is extracted from the original sample for the Extraction process. Here, drugs are retrieved from hair at significantly lower concentrations than alternative methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing positive are washed prior to confirmation where necessary. The end-to-end laboratory process, from Accessioning to Confirmation, is conducted under both the CAP (College of American Pathologists) Hair stipulation and accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
