What it is, How it works

Hair captures a lengthy timeline of drug and alcohol history by embedding biomarkers within the strands of hair as it grows. When clipped near the scalp, hair can offer up to a 3-month detection period for alcohol and drug use. Hair is straightforward to collect, relatively resistant to tampering, and convenient to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil's diameter) nearest to the scalp will yield about 100mg of hair, which is the perfect quantity for screening and confirmation purposes. For EtG, additional tests, and/or screenings beyond 10 panels, it is advisable to gather 150mg of the sample. We suggest using a jeweler's scale to measure the sample accurately. If scalp hair is not available, a similar amount of body hair can be sourced. When referring to head hair, it specifically denotes scalp hair alone. Body hair encompasses all other hair varieties (like facial or axillary hair).

Process Overview

The procedure of processing a drug test result in the laboratory typically involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample into the lab's system. This step confirms that the sample was securely sealed and transported, allocates a random LAN (Laboratory Accessioning Number), and completes any required data entry that isn't fulfilled by an electronic chain of custody process.

Screening constitutes an initial rapid evaluation for drug abuse indicators. Although Screening is a budget-friendly method to exclude drug use in most samples, a positive screen necessitates confirmation to be considered admissible in court. Samples that are initially positive in Screening require a subsequent confirmation.

When a sample is initially positive in the Screening stage, more hair is drawn from the first specimen and readied for Extraction. Within this phase, drugs are extracted from hair in much smaller concentrations than seen in other methodologies (such as urine or oral fluid), which is why hair drug screening is among the toughest methodologies to execute.

Confirmation of any initial positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All initially positive samples are cleansed before confirmation if required. The complete laboratory process, from Accessioning to Confirmation, is validated under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
