A sample of 1.5 inches with roughly 200 hairs (similar to the thickness of a #2 pencil) closest to the scalp equates to 100mg of hair, which is the optimal amount for testing. For EtG, add-ons, or assessments beyond 10 panels, it's advised to gather 150mg of hair. Using a jeweler’s scale to weigh the sample is recommended. In the absence of scalp hair, an equivalent amount of body hair is acceptable. When mentioning head hair, we are specifically referencing scalp hair, whereas body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The key stages in a laboratory's processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial procedures to integrate a sample into a laboratory’s database. This process involves ensuring the sample was correctly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and fulfilling any data entry not captured by an electronic custody system.

Screening consists of an initial evaluation for substances of abuse. Although Screening is a budget-friendly approach for excluding drug use for most samples, a positive result must be confirmed for legal acceptance. Presumptively positive samples from Screening necessitate further confirmation.

Should a sample be presumptive positive during Screening, extra hair is extracted from the initial batch and set for Extraction. This process extracts drugs from hair at lower concentrations compared to other methods such as urine or oral fluids, making hair drug screening the most complex testing methodology.

Confirmation of any positive screening finding is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a wash before confirmation if required. The entire laboratory sequence from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and conforms to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
