If scalp hair is unavailable, collecting an equivalent amount of body hair is permissible. Head hair refers exclusively to scalp hair. All other hair types (facial, axillary, etc.) are categorized as body hair.

Process Overview

The laboratory testing of drug samples encompasses four crucial steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, samples are initially processed into the lab’s system ensuring that the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and inputting any additional data not covered by an electronic chain of custody system.

Screening conducts an expedited preliminary test for substances of abuse, effectively ruling out drug presence in most samples. Positive results in Screening must undergo confirmation for legal acceptability. Presumptive positive samples in Screening necessitate secondary verification.

In cases of a presumptive positive result during the Screening phase, more hair is selected from the original sample specifically for Extraction. During this phase, drug substances are removed from hair at concentrations lower than those usually found in urine or oral fluid tests, marking hair drug screening as the most intricate procedure to conduct.

Confirmation of positive screening outcomes employs GC/MS, GC/MS/MS, or LC/MS/MS methodologies. Presumptive positive samples are pre-washed according to requirements before confirmation. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
