For EtG, add-ons, and/or tests surpassing 10 panels, 150mg is advisable. Using a jeweler’s scale is recommended for weighing. If the scalp hair is absent, a similar quantity of body hair may be used. "Head hair" refers exclusively to scalp hair, whereas "body hair" pertains to all other types (facial, axillary, etc.).

Process Overview

Among the four primary phases in lab processing for drug tests are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where the sample enters the lab’s system. This includes ensuring the sample's proper sealing and shipment, allocating a random LAN (Laboratory Accessioning Number), and conducting supplementary data entry if not supplied by an electronic chain of custody system.

Screening is a preliminary, swift evaluation for drug abuse. While Screening offers a cost-effective way to exclude drug usage in most samples, a positive result needs confirmation to hold up in court. Samples showing a positive indication in Screening mandate another level of confirmation.

If the Screening phase results in a presumed positive, more hair is extracted from the initial collection for Extraction. Here, drugs are isolated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting why hair drug testing is the most challenging method to implement.

Confirmed results from Screening are validated through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undergo washing before confirmation if necessary. The entire laboratory process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
