What it is, How it works

Hair testing has emerged as a significant method for identifying drug and alcohol consumption. Hair strands encapsulate a long-term record of substance use by embedding biomarkers within their fibers as they grow. When samples are taken near the scalp, they can offer a detection period of roughly up to 3 months for alcohol and other substances. Hair collection is straightforward, relatively resistant to tampering, and convenient for transportation.

A sample measuring 1.5 inches containing roughly 200 hair strands (comparable in size to a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal amount for testing and analysis. For EtG, additional testing, and/or examinations exceeding 10 panels, it is advisable to use a specimen weighing 150mg. We suggest using a jeweler’s scale to measure the specimen’s weight. In cases where scalp hair is unavailable, an equal amount of body hair may be used. When discussing head hair, we specifically mean scalp hair. Body hair encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into a lab's system. This entails validating the sample's sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry absent from an electronic chain of custody system.

Screening is a preliminary check for drugs of abuse. While Screening is an economical approach to exclude drug usage in most samples, confirmation is necessary for a positive screen to be legally admissible. Samples that show a presumptive positive result in Screening require further confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than other methods (e.g., urine or oral fluid), contributing to hair drug screening being the most challenging methodology.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo washing as required before confirmation. The complete lab procedure, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Extended detection window: Hair drug tests can identify drug use for up to 90 days, whereas urine tests have a more limited detection period.
  • Resistant to manipulation: Cheating on a hair drug test is extremely challenging, enhancing result accuracy.
  • Offers historical insight: It enables the detection of a usage pattern over time, not just recent activity.

Limitations:

  • Incapable of detecting recent use: Drugs become detectable in hair approximately 5-7 days after use.
  • Expense: Hair drug tests typically incur higher costs compared to other drug testing methods.
  • Result variability: Elements such as hair color and individual hair growth variations can influence drug metabolite concentrations in hair.

Note: Although commonly referred to as "hair follicle tests", the analysis targets the hair strand rather than the follicle beneath the scalp.
