Collection of hair is straightforward, relatively robust against tampering, and easy to transport.

Obtaining a 1.5-inch section of roughly 200 strands of hair (approximately the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, which is the optimal amount for screening and verification. In cases requiring EtG, additional analyses, or tests exceeding 10 panels, 150mg of the sample is suggested. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair is inaccessible, a comparable quantity of body hair might be used. "Head hair" denotes hair from the scalp exclusively. "Body hair" refers to all other hair varieties (e.g., facial, armpits, etc.).

Process Overview

The laboratory processing of a drug test follows four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the laboratory's system. This step involves confirming that the sample was properly sealed and sent, generating a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent from an electronic chain of custody system.

Screening entails a preliminary rapid assessment for substances of abuse. While Screening provides a cost-effective approach to eliminate drug use in most samples, a confirmed positive screening is necessary for court admissibility. Any samples with a presumptive positive in Screening require secondary confirmation.

For samples with a presumptive positive at the Screening stage, more hair is extracted from the original sample and prepped for Extraction. During this phase, drugs are extracted from the hair at a lower concentration than other techniques (for example, urine or oral fluid), making hair drug screening one of the most challenging methodologies to execute.

Confirmation of any positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as required. The laboratory process, from Accessioning through Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
