For EtG, additional options, or tests exceeding 10 panels, a specimen weighing 150mg is advised. It’s beneficial to weight the sample using a jeweler’s scale. If head hair is inadequate, a similar amount of body hair can be used. By 'head hair,' we mean scalp hair, while 'body hair' encompasses all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory process for determining drug test results entails four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary stage where the sample integrates into the lab system. This phase ensures the sample's correct sealing and shipping, allocates a unique LAN (Laboratory Accessioning Number), and manages any supplementary data entry absent from an electronic chain of custody.

Screening provides an initial rapid evaluation for substances of abuse. Although Screening offers a cost-efficient way to exclude drug usage in most samples, a verified positive result is necessary for legal admissibility. Any samples tentatively positive in Screening must undergo a subsequent confirmation.

Should a sample be tentatively positive during Screening, extra hair is obtained from the original sample and readied for Extraction. This phase involves extracting drugs at notably lower concentrations compared to other methods (such as urine or oral fluid), illustrating why hair drug testing is particularly complex.

Confirmation of any positive result from Screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples identified as presumptive positives are cleansed before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
