What it is, How it works

Hair captures a prolonged history of alcohol and drug intake by embedding biomarkers within the fibers of the progressing hair strand. When gathered close to the scalp, hair can offer up to about a 3-month detection window for alcohol and substances. Hair is easy to collect, somewhat hard to tamper with, and simple to transport.

A 1.5-inch segment of around 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal specimen for analysis and validation. For EtG, annexations, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest measuring the specimen on a jeweler’s scale. If scalp hair is not obtainable, a corresponding amount of body hair can be acquired. When discussing head hair, it's only the scalp hair being referred to. Body hair covers all other sorts of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample within a laboratory's system. This comprises confirming the sample was sealed and dispatched appropriately, designating a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data input not provided by an electronic chain of custody system.

Screening comprises a preliminary rapid assessment for drugs of abuse. While Screening is a cost-effective method to negate drug usage for the bulk of samples, a positive screen needs confirmation to be permissible in court. Samples that are presumptively positive during Screening necessitate a subsequent verification.

If a specimen is presumptively positive during the Screening phase, further hair is drawn from the original sample and prepped for Extraction. In this stage, drugs are extracted from hair at a noticeably lower concentration compared to other techniques (ex. urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Verification of any positive screening outcome is undertaken through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The entire laboratory sequence from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
