For EtG, add-ons, or tests exceeding 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale for weighing the specimen. If scalp hair is not accessible, an equivalent amount of body hair can be obtained. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a laboratory’s system. This process includes ensuring the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid test for drugs of abuse. While Screening is a cost-effective method to exclude drug usage for most samples, a positive result requires confirmation to be valid in legal contexts. Any samples with presumptive positives in Screening must undergo secondary confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is taken from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most complex methodology.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The whole laboratory process, from Accessioning to Confirmation, is evaluated by both the CAP (College of American Pathologists) Hair standards and accreditation to ISO / IEC 17025 standards.
