A sample of 1.5 inches, containing about 200 hair strands (roughly the size of a #2 pencil), close to the scalp, yields 100mg of hair, which is the optimal quantity for both screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, a sample weighing 150mg is advised. It is best to weigh the sample using a jeweler’s scale. Should scalp hair be inaccessible, a comparable amount of body hair can be gathered instead. The term head hair exclusively pertains to scalp hair, whereas body hair denotes all other types (such as facial, axillary, and so on).

Process Overview

The laboratory processes involved in deriving a drug test result include: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of processing a sample into the lab’s system. It involves confirming the proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data input that the electronic chain of custody system doesn’t cover.

Screening is an initial assessment to detect drug abuse. Although Screening serves as a cost-effective way to eliminate drug use for most samples, any positive results require confirmation to be legally valid. Samples presumptively positive in Screening must undergo secondary confirmation.

The Confirmation step for any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All presumptive positive samples are cleansed as needed before undergoing confirmation. The entire process from Accessioning to Confirmation is audited per the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
