When sampled near the scalp, hair offers an approximate three-month detection timeframe for alcohol and other substances. Collecting hair is straightforward, challenging to tamper with, and easy to send.

A 1.5-inch section comprising about 200 hair strands (roughly the diameter of a #2 pencil) closest to the scalp yields 100mg, which is the preferred sample size for examination and verification. For EtG, enhancements, and/or tests over 10 panels, a specimen of 150mg is advised. We suggest measuring the sample on a jeweler’s scale. If there's no scalp hair, an analogous amount of body hair can be taken. "Head hair" denotes solely scalp hair, whereas "Body hair" refers to all other hair forms (facial, axillary, etc.).

Process Overview

The drug testing procedure in the laboratory comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the introductory processing of a specimen into a lab's framework. This step verifies the sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and fills in any missing data not provided by an electronic chain of custody system.

The Screening step offers an initial rapid assessment for drug abuse. While cost-effective for eliminating most negative samples, any positive findings in Screening require confirmation for them to be legally acceptable. All samples preliminarily positive in Screening necessitate further verification.

Should a specimen yield a presumptive positive in the Screening, additional hair is selected from the original sample for Extraction. Here, drugs are extracted at much lower levels compared to other methods (e.g., urine or oral fluid), making hair drug testing exceptionally challenging.

Confirmation of any positive initial screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Every positive sample undergoes a wash before confirmation if necessary. The whole laboratory procedure from Accessioning through Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
