A 1.5-inch segment comprising approximately 200 hairs (akin to a #2 pencil) closest to the scalp yields 100mg of hair, the optimal amount for both screening and confirmation. For EtG, additional tests, and tests exceeding 10 panels, a 150mg specimen is advisable. Utilize a jeweler’s scale for precise specimen weight. If no scalp hair is present, a comparable amount of body hair can be used. "Head hair" exclusively refers to scalp hair, while "body hair" encompasses all other hair varieties (like facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial incorporation of a sample into a lab's records. This step involves ensuring the sample’s secure seal and proper shipment, assigning a unique LAN (Laboratory Accessioning Number), and filling any missing data not covered by an electronic chain of custody system.

Screening performs an initial rapid assessment for abuse drugs. Although Screening is a cost-efficient strategy to dismiss drug usage in most samples, a positive screen demands confirmation to be legally valid. Presumptive positive samples in Screening necessitate further verification.

When a sample shows a presumptive positive in Screening, additional hair is extracted from the initial sample for preparation in Extraction. During this phase, drugs are extracted from hair at a concentration much lower than other methodologies (ex. urine or oral fluid), making hair drug screening a challenging process.

Positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS methods. Preparatory washing is done on presumptive positive samples before confirmation if required. The complete lab process, from Accessioning to Confirmation, is rigorously reviewed following both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
