What it is, How it works

Hair offers an extended timeline of alcohol and drug exposure by preserving biomarkers within the fibers of the developing hair strand. When gathered near the scalp, hair can present an approximate 3-month detection timeframe for alcohol and drugs. Hair is easy to gather, somewhat resistant to tampering, and straightforward to transport.

A 1.5-inch portion consisting of roughly 200 hair strands (comparable to a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal amount for evaluation and verification. For EtG, additional tests, and analyses above 10 panels, 150mg of hair is suggested. It's advisable to measure the sample using a jeweler's scale. If scalp hair cannot be obtained, an equivalent quantity of body hair can be used. When discussing head hair, we're solely referencing scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Procedure Summary

The four primary phases in the laboratory analysis of a drug testing sample are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial processing of a specimen into a lab's system. This entails validating that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any further data input not covered by an electronic chain of custody system.

Screening involves a preliminary rapid test for substances of abuse. While Screening is an economical method to negate drug use in most samples, a positive result must be verified to be valid in legal settings. Any samples exhibiting presumptive positivity in Screening require secondary verification.

Should a specimen be presumptively positive during Screening, additional hair is collected from the initial sample and processed for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration than other approaches (e.g., urine or oral fluid), making hair drug screening one of the most complex methodologies to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptively positive specimens undergo washing prior to confirmation, if necessary. The complete laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair guideline and the accreditation to ISO / IEC 17025 standards.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
