For EtG, extra analyses, and/or assessments that exceed 10 panels, a 150mg specimen is advised. It is recommended to measure the specimen using a jeweler’s scale. If scalp hair is not accessible, a comparable quantity of body hair may be used. When mentioning head hair, it specifically refers to scalp hair alone. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial intake of a sample into the laboratory’s system. This phase includes ensuring the sample was securely sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data entry omitted by an electronic chain of custody system.

Screening consists of an initial rapid assessment for substances of abuse. While Screening is an economical method to exclude drug usage for most samples, a positive screen must be confirmed for it to be admissible in legal proceedings. Samples that are tentatively positive during Screening require further confirmation.

If Screening suggests a positive result, additional hair is removed from the initial sample for preparation in the Extraction stage. In this phase, drugs are removed from hair at significantly lower concentrations than in other testing methods (e.g., urine or oral fluids), making hair drug screening the most complex method to conduct.

Affirmation of any positive results from Screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Any samples labeled as presumptive positive are cleaned before confirmation as needed. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
