A 1.5-inch sample, consisting of roughly 200 strands (approximately the thickness of a #2 pencil) nearest the scalp, amounts to 100mg of hair, the preferred sample for both screening and verification. For EtG, additional tests, and/or tests above 10 panels, a specimen of 150mg is advised. We advocate for the specimen to be weighed using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be gathered. When discussing head hair, we mean scalp hair only, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

Key stages in the laboratory handling of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a specimen into a laboratory’s database. This step includes ensuring the sample is properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not included by an electronic chain of custody system.

Screening offers a preliminary evaluation for drugs of abuse. While Screening is an economical approach to exclude drug usage in most samples, a positive screen must be confirmed for court validity. Samples showing presumptive positivity in Screening require further confirmation.

If Screening suggests positivity, additional hair is taken from the original specimen and prepped for Extraction. In this step, drugs are withdrawn from hair at much lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug testing more complex to conduct.

Confirmation of any positive screening result is done via GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity are cleansed before confirmation as necessary. The full laboratory process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
