For EtG, add-ons, or tests exceeding ten panels, 150mg of the sample is advised. Utilizing a jeweler's scale for precise specimen weighing is recommended. In the absence of scalp hair, similar quantities of body hair can be utilized. The term head hair specifically refers to scalp hair, while body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial steps of integrating a sample into a laboratory system, confirming proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not included in an electronic chain of custody system.

Screening is a preliminary evaluation to detect drugs of abuse. Although Screening is economical for negating drug use in most samples, a positive outcome necessitates confirmation to be acceptable in legal contexts. Any samples that show presumptive positivity during Screening mandate a secondary confirmation.

When a sample indicates presumptive positivity during the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. This phase involves drugs being isolated from hair at a considerably reduced concentration compared to other approaches (like urine or oral fluid), making hair drug analysis the most challenging methodology to execute.

Confirmation of any positive outcome from Screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing ahead of confirmation if required. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair criteria and the accreditation standards of ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
