A sample measuring 1.5 inches of roughly 200 hair strands (comparable to the diameter of a #2 pencil) closest to the scalp yields 100mg of hair, which is perfect for both screening and confirmation. For extended testing such as EtG, additional tests, or panels over 10, a 150mg sample is suggested. We advise using a jeweler’s scale for weighing the hair. If scalp hair is not available, a matching amount of body hair can be gathered. When discussing head hair, we refer specifically to scalp hair only. Body hair denotes all other hair types (including facial and axillary hair).

Process Overview

The key steps in laboratory drug test processing include: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with integrating a sample into a lab’s workflow. This step ensures the sample's proper sealing and shipping, assigns a random Laboratory Accessioning Number (LAN), and handles any additional data not captured by an electronic chain of custody.

Screening performs an initial, quick assessment for drug presence. Although it's a budget-friendly method to exclude drug presence in many samples, a positive result requires confirmation for acceptability in legal settings. Any presumptive positives from Screening need a follow-up confirmation.

Should a sample get a presumptive positive result during Screening, more hair is extracted from the sample for the Extraction phase. In this step, drugs are isolated from hair at significantly lower concentrations than other methods (like urine or oral samples), which makes hair drug testing particularly challenging.

Confirmation for positive results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo a wash before confirmation if needed. The entirety of the laboratory process from Accessioning to Confirmation is examined under both the College of American Pathologists (CAP) Hair category and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
