A 1.5-inch segment of roughly 200 hair strands (approximately the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal quantity for analysis and verification. For EtG, additional tests, and/or tests exceeding 10 panels, a specimen weight of 150mg is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is not present, a comparable quantity of body hair can be used. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other hair types (facial, underarms, etc.).

Process Overview

The laboratory handling of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial intake of a sample into a lab’s system. This process includes checking proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and carrying out any extra data input not covered by an electronic chain of custody.

Screening consists of an initial rapid assessment for drug misuse. Although Screening is an economical method to determine drug use absence in most samples, a positive result requires confirmation for legal admissibility. Samples initially positive in Screening require further confirmation testing.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original specimen for the Extraction process. During this stage, drugs are retrieved from hair at significantly lower concentrations than in other methodologies (such as urine or oral fluid), making hair drug testing particularly challenging.

Verification of any positive result from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positive results are cleansed before the confirmation process as required. The comprehensive lab workflow from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
