What it is, How it works

Hair captures a comprehensive history of substance use by embedding biomarkers in the fibers of the growing strand. When obtained near the scalp, hair offers up to nearly a 3-month period for alcohol and drug detection. The process of collecting hair is straightforward, challenging to tamper with, and convenient to transport.

Gathering a 1.5-inch segment of roughly 200 strands (comparable in size to a #2 pencil) nearest to the scalp results in 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, it is suggested to gather 150mg of the specimen. Using a jeweler’s scale to measure the specimen is advisable. If scalp hair cannot be collected, an equivalent quantity of body hair is an acceptable alternative. References to head hair pertain exclusively to scalp hair. All other types of hair (facial, axillary, etc.) are termed as body hair.

Process Overview

The comprehensive lab processing of a drug test encompasses the four sequential steps of Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample within the lab's system, ensuring proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data not provided by an electronic chain of custody system.

Screening provides an initial, rapid evaluation for drug abuse markers. While Screening effectively rules out drug presence in most samples, any positive results require confirmation for judicial admissibility. Presumptive positives in the Screening phase necessitate further confirmation.

Should a sample test positive during Screening, additional hair is requisitioned from the original sample for preparation in the Extraction phase. Here, drugs are isolated from the hair at significantly lower concentrations compared to alternative methods (e.g., urine or oral fluid), rendering hair drug testing the most challenging methodology to perform.

Any positive screen result undergoes Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleansed as required. The entire laboratory process, spanning Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and certified to ISO / IEC 17025 standards.

Manuscripts in this section present advancements in forensic toxicology, and have explored the challenges posed by emerging designer drugs and strategies to combat their proliferation. The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The special issue presents also manuscripts that delve into the ethical implications of drug testing in drug facilitated sexual assaults.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
