What it is, How it works

It offers a prolonged history of substance use by capturing biomarkers within the fibers of the hair strand as it grows. When taken close to the scalp, hair can offer a detection period of up to 3 months for substances. Hair is easy to gather, not easily altered, and simple to transport.

A sample measuring 1.5 inches from roughly 200 strands of hair (around the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal amount for evaluative purposes. For EtG, additional tests, and/or panels exceeding 10, 150mg of hair is advised. It's best to weigh the sample on a jeweler’s scale. If scalp hair cannot be obtained, a comparable amount of body hair may be used. When we mention head hair, we mean scalp hair specifically. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test outcome includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample within a lab’s framework. This step ensures the sample was properly sealed and dispatched, allocates a unique LAN (Laboratory Accessioning Number), and completes any remaining data entry that was not generated by an electronic chain of custody system.

Screening provides a preliminary check for drug presence. Although Screening is economical for ruling out usage in most samples, a positive screen must be confirmed to be court-valid. Samples showing presumptive positivity in Screening necessitate a follow-up confirmation.

If Screening indicates presumptive positivity, more hair is extracted from the primary sample and readied for Extraction. In this phase, drugs are drawn from hair at notably low concentrations compared to other methodologies (e.g., urine or oral fluid), making hair drug screening one of the most complex methodologies.

Verification of any positive screening is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples with presumptive positivity are cleansed as required. The comprehensive laboratory method from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair criteria and accredited to ISO/IEC 17025 standards.
