What it is, How it works

When collected near the scalp, hair can offer a detection window of up to roughly 3 months for alcohol and other drugs. Hair collection is straightforward, relatively hard to tamper with, and easy to transport.

A 1.5-inch section consisting of roughly 200 hairs (comparable to the width of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. It is recommended to weigh the specimen using a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair may be used. When mentioning head hair, it refers specifically to scalp hair. Body hair includes other types (facial, axillary, etc.).

Process Overview

The primary stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into the laboratory’s system, which includes confirming that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any other data entry not covered by an electronic chain of custody system.

Screening is an initial rapid test for drugs of abuse. While Screening is a cost-efficient method to largely rule out drug usage, a positive result necessitates confirmation to be valid in legal contexts. Samples that are presumptively positive during Screening require further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original specimen to prepare for Extraction. In this phase, drugs are extracted from hair at a notably lower concentration relative to other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive result from Screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that presumptively test positive undergo a washing process prior to confirmation, as required. The laboratory process from Accessioning through Confirmation is rigorously reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.
