A 1.5-inch portion consisting of around 200 hair strands (about the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal amount for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg sample is advised. It is recommended to measure the sample using a jeweler’s scale. If scalp hair cannot be sourced, a similar quantity of body hair can be used. The term head hair specifically refers to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's system. This incorporates verifying the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not incorporated by an electronic chain of custody system.

Screening constitutes a preliminary rapid assessment for the presence of drugs of abuse. While Screening offers a cost-efficient method to rule out drug usage in most samples, positive screens must be confirmed to be legally admissible in court. Any samples presumed positive in Screening require further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial sample and prepared for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), marking hair drug screening as the most challenging methodology to execute.

Confirmation of any positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumptively positive are cleansed as necessary before confirmation. The complete laboratory procedure from Accessioning to Confirmation is subjected to review under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.
